AIDS (acquired immunodeficiency syndrome), which results from HIV (human immunodeficiency

virus), has attained pandemic proportions in sub-Saharan Africa where it is

spread primarily by heterosexual contact. Individual adults may have multiple sexual

partners. Many men have relations with commercial sex workers who also have multiple

clients. Because of limited access to health care, sexually transmitted diseases remain

untreated, which in turn increases the likelihood of acquiring the virus from an infected

partner (Caldwell, 1995). Large urban areas are estimated to have 20–30% seroprevalence,

and rates between 5% and 20% are reported outside of the urban areas. Two-thirds

of the world’s HIV/AIDS victims live in a region that has come to be known as the AIDS

belt which includes Kenya, Tanzania, Congo, Burundi, Rwanda, and Uganda. The government of Uganda estimated that by 1993, 1.5 million of the country’s 16.5

million Ugandans were infected with HIV. Ninety percent of the reported cases were

adults between 15 and 45 years old. AIDS is the leading cause of death for this cohort.

Although the sex ratio is roughly parity among reported cases, more females than males

are reported infected among the population under 30 years of age (Uganda AIDS

Commission [UAC], 1993).
